My AI Agent Books Appointments for Less Than a Dollar a Day

Here's the problem with leads.

They go cold. Fast.

So I built something to fix it.

60 Seconds

Within one minute of a new lead coming in, my AI texts them. Not a canned autoresponder. An actual conversation.

It asks about their symptoms. Offers a free phone consult. Books it on the calendar. All via SMS.

If they don't reply, it follows up 6 times over 14 days. Proven message sequence. Tested across 20+ clinics.

If they reply at any point, the AI switches to conversational mode. It reads the context, responds naturally, and steers toward booking.

If they ask to reschedule? Handles that too. Checks the calendar for real openings. Offers specific times. If nothing's available, it checks back in 4 hours and texts them when something opens up.

No human involvement. No leads left on read.

The Cost

Less than a dollar a day.

The AI runs on Claude Haiku. Fractions of a penny per message. Across 800+ messages and multiple clinics, the total AI cost has been under $2.

Compare that to a human appointment setter at $3,000-4,000 a month.

That's not a typo.

The Part That Gets Interesting

Every time the agent books an appointment, it analyzes the full conversation. Extracts a one-line insight about what worked. Feeds that learning into the system prompt for every other clinic.

So when a message template crushes it in Austin, the agent in Alabama gets smarter too.

The whole network improves with every single booking.

Here's what it learned so far:

  • The first message converts fastest when you ask "are you available today or tomorrow?" Direct. Specific. No fluff.
  • When leads share their pain story, that's a booking signal. Acknowledge it. Pivot to the consult.
  • Persistence works. Some leads need 5 touches over 3 weeks. Then they book.
  • "Just reply YES" dramatically increases reply rates. Make it stupid easy to respond.

The Receipts

First 3 weeks on one clinic:

Why I'm Sharing This
